SJHC COVID-19 Vaccine Clinic: Check-out
(non-clinical)
Responsibilities:
1) Monitor patients in waiting area for 15 minutes post-vaccination
2) Check patients out in CoVax database and provide patient with printout
3) Schedule follow-up appointment in 21 days (if applicable) or provide patient with
appointment for second dose.
PPE: surgical mask
Role Description:
1. Greet patient in waiting area, ensure that they have a number ticket and ask
them to sit in the next available seat. Ask patient how they are feeling and let
them know that they will need to wait 15 min before checking out.
2. Call number of next patient ticket and request that they come to the computer
check out.
3. Cross patient ticket number off number board.
4. If the patient does not experience an adverse reaction, ask the patient for their
consent form. Using the information from the consent form, search the patient’s
name in COVax.
5. Ask the patient to confirm their name and date of birth, plus one other piece of
identification (for example, their postal code).
6. In COVax, check the status bar to ensure “First-Dose Administered” Status is
complete.
7. Click the “Check-Out” button.
8. Check off “Any Adverse Events Following Immunization” and check “Email
receipt to patient”. Click “Next”.
9. To print the patient’s vaccination receipt, scroll down on the Client Profile
Page. Under “Files”, select “PDF”. Once the receipt opens, click “Download”.
10. The receipt will open in a new window. Click the print icon. Select the Bluetooth
printer and click “Print”.
11. On the printed receipt, enter the date and time of the patient’s second dose
appointment.
12. Provide the client with the printed receipt. File the consent form.
