SMH COVID-19 Vaccine Clinic: Vaccinator
Responsibilities:
1. Obtain consent, assess for contraindications to vaccination
2. Administer vaccine
3. Document
PPE: Surgical mask & Face shield
Role Description:
1) Ensure that work station has adequate supplies - see checklist.
2) Remove injection supplies from drawer: (1) vaccine syringe, (1) alcohol swab, gauze &
bandage. Gloves are not recommended unless the skin is not intact.
3) Check vaccine is not past its expiration time.
4) Raise ‘Ready’ sign to indicate to runner that they may send a patient over.
5) Greet patient as they arrive at vaccination station.
6) Complete positive identification to confirm patient with name and date of birth.
7) Request patient sit in chair, holding on to belongings. Ask them to perform hand hygiene.
8) Explain procedure, including 15-minute waiting period post-vaccination, and requirement for
2 dose in 21 days.

9) Ask patient vaccine consent form screening questions and obtain verbal consent. If pt. has
hard copy of consent form, take form and file in drawer.
a) If patient answers ‘no’ to all mandatory screening questions, proceed with vaccination.
Document screening questions, lot # of vaccine, etc. in COVax database.
b) If patient answers ‘yes’ to one or more mandatory screening questions, explain that
they are not able to receive the vaccine today and rationale. Provide patient with form
indicating they were not vaccinated and ask patient to discuss re-scheduling
vaccination with their employer and their primary care provider if they have questions
about the mandatory screening questions. Notify clinic manager or team lead of non-
vaccination.
c) If the patient is pregnant or breast feeding – use the blue MD sign to have the
physician come to obtain the secondary consent for pregnant and breast feeding
patients
10) If patient requests privacy-accompany patient to privacy screening area to vaccinate.
11) Perform hand hygiene.
12) Ask patient to expose their upper arm (preferably non-dominant arm as the site may be sore
post-injection).
13) Administer IM injection. Discard syringe in sharps container.
