SMH COVID-19 Vaccine Clinic: Vaccinator





      14) Check site for discharge. Apply bandage.
      15) Document time of injection in COVax database, as well as on ‘After-Care’ instruction sheet.
            Provide ‘After-Care’ instruction sheet to patient.

                i. Note – If a patient needs to wait for longer than the standard 15 minutes (e.g. in the
                   case that a person has reacted to a vaccine in the past and requires 30 minutes of
                   observed waiting), indicate on the “After-Care” instruction sheet the additional
                   minutes of required wait time (e.g. “+ 15 minutes”)
      16) File consent form. There is no need to document on the consent form, unless there is
            downtown and the COVax database is not working.
      17) Ask patient to dress.
      18) Direct patient to waiting area. Ask patient to perform hand hygiene prior to exiting.
      19) Don gloves and clean surface of desk with a disinfectant wipe (e.g. Clorox, Oxivir) and discard
            in garbage can.
      20) Don gloves and clean patient chair a minimum of every 4 hours or before break with a

            disinfectant wipe and discard in garbage can.
      21) Check supplies in desk drawer. If vaccine syringes need to be re-stocked, raise “Rx’ sign.
      22) If IT support is needed, raise ‘IT’ sign.
      23) If at any time you require non-urgent MD assistance, raise MD sign.
      24) At the end of your shift, bring the collected patient consent forms to the clinic office, and
            ensure that a runner has collected and returned your ipad.



     In the case of an adverse reaction:
         • Assess patient’s condition. Assist to wheelchair or stretcher as required.

         • MD is available on-site. If MD assessment is required, request check-out area runner to
            call MD to assess patient. MD should also notify manager/team lead as additional
            support and to help manage patient flow.
         • In the event of anaphylaxis:

                o Administer epinephrine intramuscularly in a dose of 0.01 mg/kg of a 1:1000
                    (1mg/mL) solution to a maximum of
                o 0.5 mg in adults in the anterolateral thigh and seek medical attention

                    immediately by activating a Code Blue (as per Unity Health COVID Vaccine Clinic
                    Medical Directive). Usual dose is 0.3 mg – 0.5 mg IM.
         • In the event of a Cardiac/Respiratory Arrest/Medical Emergency, call a Code Blue. Also
            request runner call MD STAT to assist, and call Team Lead to assist and manage patient
            flow. CPR should only be started after N95 mask has been donned.
