absence by contacting your supervisor.  If the team member is going to be out more than one
              day, she or he is required to call every day.  A team member’s supervisor may request a doctor’s
              note for an absence greater than three days.  The team member's failure to report her or his
              absence, may result in disciplinary action up to and including termination.

              Paid Time Off Limits

              Orientation Period

              Team  members  may  be  denied  approval  for  Paid  Time  Off  during  their  90-day  orientation
              period.

              Maximum Balances

              Earned but unused Paid Time Off balances may not excess 1.5 times the team member’s then
              current annual accrual rate.  That is, team members with tenure of less than 4 years may not have
              unused Paid Time Off balances in excess of 216 hours and team members with 4 or more years
              of  service  may  not  have  unused  Paid  Time  Off  balances  in  excess  of  276  hours.    Once  the
              maximum hours have been reached, no additional Paid Time Off hours may be earned until the
              balance is below the maximum.

              Advanced Paid-Time Off

              A  supervisor  may  approve  the  advancement  of  Paid  Time  Off  hours  in  excess  of  accrued
              balances; however, such advancement of PTO shall generally be limited to time expected to be
              earned in the then upcoming two-month period.

              Transition/Carry-Over Balances

              Effective October 1, 2017, all Team member accrued but unused time off balances for vacation
              and personal time have been converted to Paid Time Off hours. Sick leave up to a maximum of
              64 hours has been converted to Paid Time Off; sick leave hours in excess of 64 hours have been
              converted to a pool of Carry Over Sick Leave.

              Carry Over Sick Leave

              Carry  Over  Sick  Leave  may  be  used  only  when  the  team  member's  illness  or  incapacity
              interferes with or makes it impossible for the team member to satisfactorily perform her or his
              assigned duties.

              Absences that are due to illness must be reported 1 ½ hours before your shift on the first day of
              absence by contacting your supervisor.  If the team member is going to be out more than one

              Use of Paid Time Off

              Accrued but unused PTO hours must be used for any full day absence (i.e. leave without pay
              may not be taken for any full day absence when a team member have an available PTO balance).
              If the team member does not have any available PTO hours an Unpaid Time Off form should be
              submitted.  A time off form must be submitted for all absences.
