WHAT WAS THE PROBLEM?
Recipient card was stored according to blood grouping of the patient. Hence, staffs tend to miss the card if the blood grouping was wrongly
written on the form.
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
1. Recipient card was stored and arranged according to the last 4 digits of the Identity Card number (according to numerical order).
2. Recipient card for thalassemia patient with frequent transfusion was stored in a separate drawer.

WHAT WAS THE BENEFITS?
1. More systematic and organized.
2. Reduced the chance of transfusion error in patient with previous record of blood transfusion.
3. Easier for staff to look for the card in previously transfused patient.
