MODULE 3 OSEP Reporting Procedures


               At the end of each OSEP course, there are reporting requirements that an OSEP Educator
               must comply with. The records that should be kept include:

                    1.  Completed OSEP Educator Assessment Checklist
                    2.  Session plans of all OSEP Educators

                    3.  Correctly named excel database for the course

                    4.  Summary of course evaluation with all evaluation forms
                    5.  Risk minimisation matrix

                    6.  Photos of participants

                    7.  Photos or videos of evidence of written work
                    8.  OSEP Individual Program Report

                    9.  QA Report (by the Master Educator, if applicable)

                    10.  Any other support documents e.g. attendance records, participant profile analysis
                        spreadsheet
