Documents that staff will need to use (and store) as risk management tools
include:
• a risk audit check list to be completed at the start of each shift. This will identify
any potential risks and allow the staff to take corrective action. Sections within
this check list will include:
– equipment check
– floor space check
– first aid equipment check
– staff on duty
– forms cabinet
– lighting
– ventilation
– phone check
– signage visibility check
– appointments.
• injury report sheet
This will specifically record the circumstances surrounding any type of injury that
occurs in the centre to staff or clients.
• incident report sheet
This can record incidents such as equipment failures and actions taken, etc.
• client feedback forms
It is the staff’s responsibility to implement all aspects of the plan and to be involved
in its review and modifications on an ongoing basis. The staff must ‘own’ the plan.
The plan must be useable. There is no point having an elaborate plan that sits on
the shelf. It must be a living document!
