MODULE 3 – OSEP Reporting Procedures
It is a must to have these reports comprehensively completed as each day progresses. You
are to sign off on the reports so coordination with the Course Administrator and
Coordinator is of vital importance.
At the end of this course, there are reporting requirements that an OSEP Master Educator
must comply with. The records that should be kept include:
1. Completed OSEP Educator Assessment Checklist
2. Session plans of all OSEP Educators
3. Correctly named excel database for the course
4. Summary of course evaluation with all evaluation forms
5. Risk minimisation matrix
6. Photos of participants
7. Photos or videos of evidence of written work
8. OSEP Individual Program Report
9. QA Report for OSEP Educators (by the Master Educator)
10. Any other support documents e.g. attendance records, participant profile analysis
spreadsheet
