Survey Meeting Reminders!
• If any questions appear to make the person
uneasy, or if you know from family or staff
that a particular line of questions may be a
“trigger” do not ask those questions
• Many responses – particularly Community
Inclusion – will be affected by COVID
• Participants should answer questions to the best
of their ability based on their current experience
• Always use the “99-Don’t Know/Unclear”
response when needed
• PRACTICE!!
