Section I
Section I can only be completed in a face-to-face meeting with the person receiving services. A translator/interpreter may assist with completing
Section I as long as the surveyor is confident that the translator/interpreter is conveying the appropriate questions and responses between the
surveyor and the individual. Surveys should be conducted one-on-one with the individual unless an additional person is required (e.g., for
behavioral support) or the person requests an additional person be present; surveyors should attempt to limit the number of people present at
the meeting.
The meeting should take place in an area of the person’s choosing that is quiet and has few distractions. Surveyors should remind the person:
• The purpose of the survey
• That completing it is voluntary
• There are no right or wrong answers
• That answers will not be shared with others, and services will not be affected
As a reminder, there is proposed content for the introduction in the survey (preceding Section I).
Surveyors should review the Pre-Survey information prior to the start of the meeting and fill in information where appropriate throughout the
survey. Areas in the survey where pre-survey information is relevant are indicated with a
