Section II
Section II of the survey should be completed through a face-to-face survey with the person being surveyed; however, this section may be
completed in part or in full by a proxy respondent. The proxy must be someone who knows the person well and is familiar with the person’s
schedule/routine. The survey should be completed with the survey respondent and proxy together (in person) as often as possible. Proxy
surveys by telephone are only allowed if the surveyor has met with the individual being surveyed and in the rare instance when an in-person
survey cannot be arranged with the proxy. If a proxy conversation will be occurring by telephone, please consult with HSRI for guidance.
For all questions, the surveyor should mark the identity of the respondent to the question.
For each question, only one respondent type is allowed to be marked.
• If the individual and another person answer the question, mark that the individual answered.
• If more than one person is completing the section and there is disagreement between respondents, the surveyor should follow up to get
a consensus. If a consensus cannot be reached, the surveyor should code the question ‘99-Don’t know, unclear response’.
• If a family member who is also the person’s staff is answering, code the respondent type as ‘family’.
