                                                                                      COMMUNITY INCLUSION

                                         Overall instruction:
                                            • Be sure that answers only reflect going out in integrated settings (where there are people without disabilities).
                                            • To determine the number of times a person went out in the past month, the surveyor can ask about the past week and then multiply
by four or ask about the past two weeks and multiply by two. Approximations are ok.
                                            • These questions are referring to the actual past month, not a typical month.
                                            • If the person goes out with different people for community activities, ask about who the person goes with most often.
