                                              Now I am going to ask some questions about life during the COVID-19. Sometimes it’s called Coronavirus.  COVID-19 first appeared
                                              and started making people sick in the US around February 2020. Many people had to stay home and not see friends and family to

                                              Because of COVID-19, starting around March 2020, a lot of people had to make changes in their daily life and routines. For example, a
                                              lot of people had to stay home, they couldn’t go to their work or day program and couldn’t go out to see family and friends or go into
                                              the community as much.
                                              We’ll call that time “COVID time.”

                                              For some people, COVID time might still be happening, even just in part.
                                              And for some other people, COVID time might be completely over.
                                              Note to surveyor: Please try to adapt wording of questions to reflect the current experience of the person being surveyed.  For example,
                                              if the person is still experiencing effects of the coronavirus lock-down, and the written question says: “During COVID time, did you
                                              use….” please change the wording to “During COVID time, have you used….”
