        Benefit eligibility criteria

        The following are the new classifications of employees and the benefits each group is eligible
        for as of 1/1/2022:



        Full-time regular employees consistently scheduled 30+ hours per week


        • Eligible for all GESMN benefits which include medical, HSA, dental, vision, life, LTD, STD,
           Dependent Care FSA, critical illness hospital, accident, legal, pet insurance, and employee
           assistance program


        • Eligible on the first of the month after 30 days of full-time regular status




        Part-time variable employees who average 30+ hours per week under ACA measurement

        •    Eligible for medical insurance with the option of an HSA as well as GESMN’s employee

             assistance program if averaging 30+ hours per week after one of the following periods:
               •    New hires are measured 12 months after date of hire
               •    All employees are measured annually each October
               •    HR will notify employees in writing if they meet eligibility



        All other part-time employees

        • Eligible for GESMN employee assistance program
