        Other changes and important things to note

         Elections you make during open enrollment will become effective January 1, 2022. To keep
         up with evolving trends, below are changes you will see in this year’s benefit package:



         Engaged rates
                   •    The program requirements are changing slightly to be more employee-driven in
                        2022. More details to come in January 2022.
         Medical plan
                   •    GESMN plans will remain with HealthPartners with no increase to premiums
                   •    GESMN will continue to offer two plans with no changes:
                       $3,500 – 80% HSA plan – Open Access Network
                       $2,800 – 80% HSA plan – Achieve Network
         Nice healthcare

                   •    Nice Healthcare will continue to be offered as of January 1, 2022.
         Dental plan:
                   •    Dental plan will transition to MetLife on January 1, 2022.
         Vision plan:
                   •    Vision plan will remain with EyeMed with no increase to premiums.
         Ancillary benefits
                   •    Life/AD&D, voluntary life/AD&D, short-term disability, long-term disability and
                        accident, critical illness, and hospital insurance will transition to MetLife on
                        January 1, 2022
                   •    Life & AD&D 100% paid by GESMN
                   •    Short-term and long-term disability 100% paid by GESMN

                   •    Dependent care FSA 100% employee paid
                   •    Voluntary life and AD&D 100% employee paid
                   •    Voluntary hospital, critical illness, and accident insurance 100% employee paid
                   •    Employee assistance program 100% paid by GESMN
                   •    Pet insurance 100% employee paid
                   •    Legal 100% employee paid



         Current voluntary life plan amounts will carryover to MetLife without a statement of health
         if actively elected at open enrollment. If you have more than one voluntary life plan, the
         amounts will be combined into one plan with MetLife. If you have a voluntary life only plan,

         it will be converted to a voluntary life AD&D plan with an adjusted premium.
