Financial Protection Benefits




             Short-Term Disability

             Company  paid  short-term  disability  insurance  replaces  a  portion  of  an  employee’s  income
             (based on years of service) for the first 90 days of an absence due to illness or injury. Full
             time employees become eligible on their 91st day of work and disability payments follow a 14
             day waiting period.


             Long-Term Disability

             Company paid long-term disability insurance replaces 60% of base salary due to an illness or
             injury  that  extends  beyond  90  days  for  full  time  employees.  New  hires  have  the  option  to
             purchase supplemental coverage that would replace an additional 15% of their base salary.
































              Life Insurance
              Life and accidental death insurance is provided free to full-time employees. The benefit is
              1.5X base salary up to a $750,000. Employees may purchase additional life insurance for
              themselves and for their spouse, up to a $500,00 maximum. Employees may also purchase
              child life insurance of $10,000 for children up to age 26.


              Cancer Insurance

              Employees can purchase AFLAC cancer insurance for themselves, their spouse and their
              children up to age 26. AFLAC cancer insurance provides financial benefits in the event of a
              cancer diagnosis and treatment.
