Changing Your Coverage

It is your responsibility to contact Human Resources and make your
necessary changes within 30 days of the qualifying life event. Your
change, if approved, will be effective on the date of the qualifying event.
Failure to contact Human Resources and request your changes within 30

days will prohibit your ability to change your medical, dental, and vision
election(s) until the next annual enrollment.
