Beneits Program




UnitedHealthcare Choice HMO Plan

(Available to California Residents Only)
UnitedHealthcare’s HMO Choice Plan features a narrowed network

of healthcare providers at your disposal. But unlike traditional HMO
plans, you are not required to designate a Primary Care Physician (PCP)
when you enroll in the plan. You have the freedom to seek services
from any Choice network provider without needing referrals or
authorization from a PCP. Services rendered outside the UHC Choice
network are not covered, except in emergency situations. Otherwise,
beneits are only available when services are rendered by a Choice

network provider.


Kaiser Permanente HMO Plan
(Available to California Residents Only)

Kaiser Permanente is a fully integrated, private medical organization
including physicians, pharmacies and hospitals. All medical services
must be obtained through Kaiser Permanente providers, except in
emergency situations. Shortly after enrollment Kaiser will send you a

“Welcome Kit” with instructions on how to designate a primary care
provider (PCP). Kaiser Permanente will assist you in the selection of a
primary care provider that meets your needs, from location, specialty,
gender, language or personal interests. You may change providers at
any time. Specialists referrals are required for some services.
