UnitedHealthcare Choice Plus POS Plan
The UnitedHealthcare Choice Plus POS Plan features a national Medical
network of physicians and hospitals that have agreed to a discounted
payment schedule. The POS plan allows for more lexibility, but more New Medical Carrier
responsibility on your part. You are not required to select a PCP— Effective July 1, 2015, we will transition
select a doctor of your choice at the time of service. You may access our medical insurance provider to

account in order to use it. If you don’t have funds in your HSA and
must pay out-of-pocket with cash or another bank account, you can
reimburse yourself from the HSA at a later date when the funds are
available.
