PORTABILITY AND EXTENDED CONTINUATION

CALCULATING PREMIUM FOR CRITICAL, ACCIDENT AND
HOSPITAL INDEMNITY COVERED EMPLOYEES

Rates for Critical Illness are based upon the employee's age, tobacco
usage (if applicable), and who is being covered by the plan. Rates will
be based on a traditional monthly billing cycle.

Rates for Accident are based upon who is being covered by the plan
(employee only, employee and spouse, employee and children, and
employee and family) as well as the level of coverage chosen. Rates will
be based on a traditional monthly billing cycle.

Rates for Hospital Indemnity are based upon who is being covered by
the plan (employee only, employee and spouse, employee and children,
and employee and family) as well as the level of coverage chosen. Rates
will be based on a traditional monthly billing cycle.
