POLICYHOLDER-REQUESTED CANCELLATION



In the event that you decide to cancel your insurance policy, you must notify us in writing. Failure to do so may
result in additional premium payments due to The Hartford for extended liability. Please send written notification
to your field sales/service representative from The Hartford to terminate your Critical Illness, Accident and/or
Hospital Indemnity coverage.

Premiums are due for any coverage extended between the premium due date and the date of cancellation. Please
be aware that you'll be billed for insurance coverage up to the date of the letter notifying us of your cancellation.
If a policy is cancelled, coverage ceases and no new incurred claims will be payable under the policy.


ENROLLING EMPLOYEES, TERMINATING EMPLOYEES AND REPORTING CHANGES
If you're utilizing a file feed, all changes must be reported on the feed. No manual changes can be
accepted for a case on a file feed. For list bill customers, all census changes are reported by the customer
on www.employerview.com , via phone to the Customer Contact Center, or via email or fax.
