REPORTING CHANGES

As the employer you're also responsible for informing us when a dependent child no longer meets the

Age Banding (Step Rates): All employees’ age changes will be reflected based on the effective date
chosen by the Policyholder. The effective date options are:
• The Anniversary Date following the employee’s birthday (all changes take place on the Policy
Anniversary Date)
• The first of the month following the employee's birthday;
• Other specified date (e.g., January 1, August 1, etc.).
