ILLNESS, ACCIDENT AND HOSPITAL

INDEMNITY LINES


NON-CONTRIBUTORY COVERAGE
For this type of coverage, the most crucial aspect of the enrollment process is the
designation of a beneficiary (Accident and Hospital Indemnity). This may be done
in the appropriate section of the enrollment form. The employer pays for coverage
when identified as non-contributory.

Note: Benefits are paid to the estate in the case of a Critical Illness claim for a
deceased insured.


CONTRIBUTORY COVERAGE
Employees need to elect coverage and authorize payroll deductions to pay for
such coverage.



ANNUAL OPEN ENROLLMENT DEFINITION

Annual Open Enrollment is a scheduled period for initial
and subsequent enrollments which occur annually
thereafter. Annual Open Enrollment necessitates that
certain employer characteristics are met and that
defined enrollment experience practices are agreed
to be implemented by the employer.













For employers with 500 lives
Personalized enrollment
experience is created or more, The Hartford benefit
either online or through counselors may be utilized at each
paper forms. employer location with at least
200 employees on-site.




Mandatory employee group meetings and Benefit
75% Fairs are conducted; employer support of enrollment
events is essential and attendance by at least 75% of
eligible employees is expected.
