Basic Protection Program (New Employees Only) Basic
Guardian’s basic protection package combines multiple insurance Protection
offerings for a bundled price. This plan includes term life insurance,
accidental death and dismemberment, long term disability, and a critical Program
illness beneit. The coverage levels are already built in to the program
with pre-determined rates based on your age. Speciic beneit coverage
and cost details are available through the online enrollment system.
Coverage Details
Term life $25,000 of term life insurance amount is reduced by
35% upon attainment of age 65 and an additional
25% of the original amount upon attainment of age
70
Accidental death and $75,000 beneit in the event of an accidental death
dismemberment
Long Term Disability
Deinition of disability Two year own occupation/ADL disabled thereafter,
with zero day residual beneit
Monthly beneit 66 2/3% of an employee’s salary to a monthly
maximum of $1,000
Elimination period 90-days for accident or illness
Duration of beneits To age 65
Pre-existing condition 3 months prior/12 months thereafter

Critical Illness
Critical illness $5,000 lump sum beneit
Covered conditions Cancer (type 1), heart attack, kidney failure, organ
transplant, stroke (100% irst occurrence, 50%
second occurrence) cancer (type 2), coronary artery
bypass graft (25% irst occurrence, 0% second
occurrence)
Total amount payable During an insured’s lifetime, this plan will not pay
more than 150% of the beneit amount for all covered
conditions combined
Beneit reductions 35% at age 65, 60% at age 70, 75% at age 75, and
85% at age 80
Pre-existing condition 3 month look back period, 12 month exclusion period
