Coverage provided by Americare in conjunction with Principal
Financial Americare matches 50% of the irst 3% of the employee’s Savings Plan
salary contributions. Various investment options are available. 401(k)

Eligible Employees
X After 1 year of employment

X Must be 21 years of age
X Must have worked 1,000 hours in the past year

Plan Highlights
X Pretax contributions are made through payroll deductions

X Employee may contribute percent of wages or set dollar amount
per pay period

Z 2016 contribution limit is $18,000
X Company matches 50% of irst 3% of employee salary
contributions

X Employer match is 100% vested after six years

X Various investment options available
X Money cannot be withdrawn at any time without tax due and IRS
penalty

X Employees over age 59½ may request an early withdrawal which
will be taxed but not penalized by the IRS
