Accidental Death and Dismemberment AD&D Plan Benefit
Enhancements
Basic AD&D Insurance Seat belt beneit—additional 10%
Eligible employees will receive an AD&D insurance beneit of two times beneit in event of death while
wearing a seat belt; $10,000
your salary. These beneits are provided at no cost to you and will be paid maximum
to your designated beneiciary in the event of a claim. All beneit amounts Waiver of premium—family
over $400,000 are subject to approval. coverage continued for 12 months
following employee’s death;
Voluntary AD&D Insurance—Employee Only conversion available
The plan allows you to purchase voluntary accident insurance on yourself
with pre-tax dollars. The Plan provides payment to your beneiciary in the
event of your accidental death, or to you in the event of your paralysis or
dismemberment. Beneits are paid in addition to other policies in effect
at the time of the accident. You may purchase coverage in amounts of
$50,000, $100,000 or $250,000.
Voluntary AD&D Insurance Family Plan
If you elect Voluntary Accidental Death & Dismemberment, you may
also cover your family on a pre-tax basis. The Family Plan maximum
options are based on the employee’s beneit at time of loss as listed.
Your spouse may be covered under the plan until he/she attains age
75. Children may be covered up to age 26, regardless of student status.
Coverage under both the employee only and family plans may be
converted to individual policies. The beneit levels and monthly costs are
printed on your enrollment form.
