2019 BENEFITS ENROLLMENT
2019 Tobacco Surcharge
As you enroll for your 2019 benefits, you must indicate whether or not you
and your medical plan covered spouse are tobacco users. Your tobacco
attestation will also determine the basis of your critical illness premium if
elected. If you or your spouse is a tobacco user you will pay an additional
$23.08 per pay period for either of the medical plan options. If both you and
your spouse are tobacco users, you will pay an additional $46.16 per pay
period for either medical plan option. Each non-tobacco user will save $600
a year in medical premiums, a total of $1,200 if both you and your enrolled
spouse are tobacco free.
If you and/or your covered spouse indicate you do not use tobacco, you and/
or your covered spouse,will be required to complete a cotinine test between
now and December 15, 2018 in order to receive the discounted medical plan
premiums.
If you and/or your covered spouse are currently tobacco users you will be
subject to the tobacco surcharge, however; you and/or your covered spouse
will be provided the opportunity to complete an 8-week tobacco cessation
program which is provided at no cost. An announcement will be made after
the first of the year with the dates and times of the tobacco cessation classes. If
you and/or your covered spouse complete the program by June 25, 2019, you
will receive a one-time taxable refund for the January through June tobacco
surcharge paid, and you will pay the discounted medical premium for the
remainder of the plan year.
If you do not complete the tobacco cessation program by the June 25, 2019
deadline, you will not be permitted to qualify for the discounted medical
premium until the 2019 annual enrollment period.
If you and/or your covered spouse begin to use tobacco products after the
first of the year, you must notify Human Resources and you will begin to pay
the tobacco surcharge the following pay period. Franke reserves the right to
test for cotinine if there is “reasonable suspicion” including observation or
reporting by others. An inaccurate representation of your tobacco status is
a violation of company policy and may result in collection of the applicable
premium surcharge by the plan, cancellation of your medical coverage
under the terms of the plan, and/or disciplinary action up to and including
termination of your employment.
