Voluntary Term Life Insurance
Voluntary term life insurance is available for yourself, your spouse,
and dependent children. If you wish to elect more than the
guaranteed issue amount, an evidence of insurability form will need
to be completed. This form will be available when you go through
the online enrollment site.
Employee
Maximum beneit Lesser of 5x annual salary or $500,000
Guaranteed issue * $110,000
Spouse
Maximum beneit Lesser of 5x employee’s annual salary or $500,000
(cannot exceed employee-elected amount)
Guaranteed issue * $25,000
Child(ren)
Maximum beneit $10,000 (one rate, regardless of how many children
are covered)
* No medical questions
For coverage to be effective for yourself, your spouse, and/or your
dependent children, you must be actively at work on the date your
beneits become effective. Your spouse and dependent child(ren)
must be performing the task of similarly situated spouse (children).
If not, coverage will not be effective until you return to active
employment or your spouse and/or children are performing normal
duties.
