PAPER: X-RAYS: ELECTRONICS:
Bring your old files in Bankers Boxes Drop off your x-ray films to be Drop off your old

Sell your dental scrap metal. Precious metals purchased by Jeff Lasky of Garfield Refining. Payments will be
made by check after the event.

Please note: Destruction will occur off site and a Certificate of Destruction will be sent after the event.

Completed forms must be submitted to NCDS
by 9:00am on September 10, no exceptions.
