Subsequent Applications:
After the initial application is approved, an employee is required to obtain approval signatures for each
new semester, term, or set of courses from that point forward.
If an employee decides to change his/her educational program, he or she must resubmit a new
application and is subject to the full approval process.
Requesting Reimbursement:
Once the course(s) have been successfully completed per the terms of the plan, an employee should
submit all grades to Human Resources along with the Application for Educational Assistance and
Reimbursement showing the required approval signatures obtained prior to the start of the course(s).
The following information should be included with a request for reimbursement:
1. Receipts or other written statements of independent third parties describing the amounts,
dates, and nature of the expenses for which benefits are requested;
2. Evidence of the employee’s successful completion of the educational course; and
3. Statements of financial assistance received with respect to the educational course (if any).
4. Additional information may be required, as deemed necessary, to calculate benefits under
this plan. Unless an extension has been granted, such additional information must be submitted
within thirty (30) days of the request.
