Just as it sounds, Supplemental Medial Plans – Accident and Critical Illness insurance can help you pay for costs you
may incur after an accidental injury, illness or hospitalization. These plans are 100% voluntary.
Supplemental Medical Plans pay a fixed, one-time benefit amount which you can use for any purpose you like. It can
help pay for expenses not covered by your health care plan (such as your deductible or copays), lost income, childcare,
travel to and from treatment, home health care costs or any of your regular household expenses.

Eligible Expenses
• On his way to work, John was in a car accident.
Emergency Room Visits • He was transported by ground ambulance to the
emergency room and admitted to the hospital.
Medical Exams – • He had a dislocated hip and spent five days in
