Safety Memo #29 Root Cause Analysis

[ Root Cause Analysis ]
In order to keep on top of safety on the jobsite, we need to learn from past incidents and do our best to make
sure they don’t repeat themselves. We will now use the Root Cause Analysis (RCA) as part of our incident
coordination to find any underlying circumstances that may have caused an accident or near-miss (near
misses were previously defined in Safety Memo #25) and detail what actions will be taken to prevent similar
incidents in the future.
Please note that the Root Cause Analysis will be required as part of the follow-up to all incidents and
near-misses from now on.
