healthy and feel secure while maintaining a quality work/life balance.
SIPS Consults, Corp. is pleased to offer four medical options to meet your individual needs,
including a Health Savings Account (H.S.A) qualified plan. SIPS pays the majority of the
cost of full-time employee’s base plan and a portion of your dependent’s medical plan.
We provide our full-time employees with low cost voluntary Dental plans, Vision, Life/AD&D,
and Short Term Disability. In addition, SIPS pays 100% of your Basic Life/AD&D and a Long-
Term Disability, NY-STD Plan, Employee Assistance Pan (EAP), Travel Assistance. We also
offer a Voluntary Critical Illness and Accident coverage for employees and dependents.
Our Voluntary dental has been changed to Humana that includes ADULT Orthodontics &
UNLIMITED Annual Maximum Benefits for Preventative, Basic and Major benefits.
Please retain a copy of this guide in order to review coverage benefits, cost information,
and retain necessary contact information. This booklet will not serve as an ID card or
verification of benefits at a provider’s office, but can provide assistance if needed. For
more information and to review the Glossary of Health Coverage and Medical Terms, you
can log into your SIPS Consults, Corp. Benefit Website 24/7 with your personal ID and
password listed on page 26.
