                               PERSONNEL BENEFITS AND INSURANCE PLANS


                  All full-time, regular employees and, as mandated under the Affordable Care Act, part-time
                  employees  who  work  an  average  30  hours  per  week  or  more  per  year,  are  eligible  to
                  participate in Molzen Corbin’s medical coverage insurance. Certain other types of additional
                  group insurance are available to all full-time, regular employees. Dependent coverage is
                  available on an optional basis at personal cost. Eligible employees who wish to participate
                  in  group  health  insurance,  dental  insurance,  or  disability  insurance  should  consult  with
                  Human Resources to obtain written information about this offer of coverage. Each is covered
                  in detail in a separate brochure that is part of your employee information packet. Some of
                  the benefits discussed below are mandated by Federal and/or State law.

                  A.     SOCIAL SECURITY


                         The federal social security program provides certain old-age and disability assistance
                         benefits.  Molzen Corbin contributes an amount as required by federal law.

                  B.     WORKER’S COMPENSATION

                         In  accordance  with  federal  and  state  laws,  Worker’s  Compensation  Insurance  is
                         carried on all employees by Molzen Corbin at no expense to you to protect you in
                         the event of your being injured while on the job.  The terms of this coverage require
                         that an injury report be filed immediately.  Such injury reports shall be filed with the
                         Human Resources Department.

                  C.     GROUP MEDICAL INSURANCE


                         All full-time, regular employees and, as mandated under the Affordable Care Act,
                         part-time employees who work an average 30 hours per week or more per year, are
                         eligible for  group medical  insurance  unless a waiver of coverage and reason  for
                         waiver is signed by the employee. Currently, Molzen Corbin pays a percentage of
                         premium  for individual medical  and dental coverage  and this  percentage  will  be
                         reviewed/amended on an annual basis.  Employees who choose to accept the offer of
                         coverage  for  medical  and  dental  insurance  through  Molzen  Corbin  will  pay  the
                         balance of the premium to cover themselves and/or dependents. These amounts are
                         automatically deducted from employee paychecks.
