Cigna - Voluntary Term Life Insurance Options Include:
Employees in increments of $10,000 up to the lesser of 5 times their salary to a maximum of $500,000.
Amounts above $200,000 will require Evidence of Insurability.
Spouse in increments of $5,000 to a maximum of $250,000, not to exceed 50% of the employees
benefit. Amounts above $30,000 will require Evidence of Insurability.
Children in increments of $1,000 to a maximum of $10,000. Under ae 6 months, the maximum benefit
is $1,000
Accidental Death & Dismemberment insurance is included

After the first 60 days of hire you are welcome to apply anytime throughout the year! Simply complete
the application and short medical form and we will take care of the rest. Once you have been
