result in denial of benefits. STD benefits will run concurrent with available
FMLA leave if you are eligible for FMLA leave.
You must notify your supervisor as soon as you become aware that a period
of disability is pending or within three (3) days that a period of disability has
occurred. HROD will provide you with the required documentation to apply
for benefits.
2. Unpaid Personal Leave: Personal leaves of absence may be granted when
unusual or unavoidable circumstances require an employee's absence.
Full-time employees may request a personal leave of absence without pay or
company-paid benefits for a period not to exceed thirty (30) days after six (6)
months of service. Requests should be submitted in writing to the supervisor
as far in advance of the anticipated leave date as possible. The request must
state the reasons and duration of the leave. The decision to approve a
personal leave is in the sole discretion of the Company and will include
consideration of your overall work record, as well as the urgency of your
situation and the work load of the work group. You are required to exhaust
available vacation and personal hours at the start of the leave, up to
maintaining a reserve of 40 hours. Working for another employer while on
unpaid personal leave is prohibited.
3. Military Leave: An employee shall be granted a leave of absence for the
initial active military service or reserve military training in accordance with
requirements of Federal and State legislation. Employees are required to
submit a copy of their official orders or a letter from their commanding officer
stating the length of duty. Employees who take military leaves covered
under Uniformed Services Employment and Reemployment Rights Act
(USERRA) are provided with the benefits, job eligibility and other rights
covered under the Act.
4. Your Rights Under the Family Medical Leave Act of 1993: IV. 7. FMLA
requires Covered employers must grant an eligible employee up to a
total of 12 workweeks of unpaid leave during any 12-month period. See
the FMLA Policy for additional details regarding your rights under
Family and Medical Leave Act of 1993.
7. VACATION
Vacation time is granted to all regular status employees per the schedule below. Paid Time Away
vacation offered in the first year is not payable upon termination of employment with from Work
