Cancer Select Plus

Almost everyone knows someone that has been affected by cancer in some way……. do
you? With the group voluntary cancer coverage, you are provided with a cash benefit for
cancer and 29 other specified diseases. Your major medical coverage with UHC does not cover
the household bills that are still piling up while you are dealing with treatments and other
appointments that come along with such a diagnosis. This benefit provides:
 Coverage for the entire family Wellness Screening benefit (see brochure for details)
 Waiver of premium after 60 days of the total disability due to cancer as long as the
disability lasts
 12/12 Pre-existing condition limitation applies
o Conditions that have been diagnosed and treated 12 months prior to effective
date of coverage will not be covered for the first 12 months even if they render
the insured disabled.
 Coverage is completely portable at the same rates

In most cases the wellness reimbursement benefit covers almost your entire annual premium
cost.
