Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plan
offered at Omega Construction, you will receive an ID
Medical/Prescription Drug card from United HealthCare. You may request
additional cards by contacting United Healthcare
customer service at 1-866-633-2446.
If you enroll in the Voluntary Dental plan offered at
Omega Construction, you will receive an ID card from
Voluntary Dental Companion Life. You may request additional cards by
contacting Companion Life customer service at 1-800-
753-0404.
If you enroll in the Voluntary Vision plan at Omega
Construction, you will receive an ID card from
Voluntary Vision Community Eye Care. You may request additional
cards by contacting Community Eye Care customer
service at 1-888-254-4290.
If you enroll in the Flexible Spending Account plan at
Omega Construction, you will receive a debit
MasterCard from ProBenefits. You may request
Flexible Spending Account
additional cards by contacting Probenefits.com or by
contacting ProBenefits customer service at 1-888-722-
-8382.
