Voluntary Disability Benefits
Short Term Disability
Paid by employee, on a post-tax basis.
This benefit is payable beginning on the 15th day of disability due to an
accident or an illness and pays a maximum of 11 weeks, as long as you
meet the definition of disability.
There are two options available:
o Option 1: Provides 60% of your base pay up to $500 per week.
o Option 2: Provides 60% of your base pay up to $2,500 per week.
As of April 2018, the STD rate remains at $0.20 per $10 of weekly benefit.
Long Term Disability
Paid by employee, on a post-tax basis.
This coverage provides income replacement equal to 60% of insured’s basic
monthly earnings to a maximum of $6,000 per month in the event you are
unable to work due to a disability as outlined in the contract.
Elimination period of 90 days applies (no benefits paid during this time).
Definition of disability is 24 Months Own Occupation with 0 day residual.
Policy includes a 3 month look back for pre-existing conditions. Disabilities
that occur with the first 12 months due to a pre-existing condition are
excluded.
LTD current rate is $0.43 per $100 of monthly covered payroll.
Please note, for all of Unum Voluntary Benefits, guaranteed issue is only available if you enroll when
first eligible. If you do not enroll when first eligible you will need to complete health questions, known
as Evidence of Insurability (EOI), to be reviewed and approved/denied by Unum.
