Basic Life/AD&D





                     These benefits are 100% paid for by RS&A.
                     Benefit amount per employee is $50,000.
                     Term life benefit reduces as you age with the first reduction at age 65 to 65%, at
                       age 70 to 50% and benefit terminates at retirement.
                     Includes conversion and portability
                     Dependent spouse coverage benefit amount is $7,500.
                     Dependent child coverage benefit amount:
                          o  Live birth- 6 months: $1,000
                          o  6 months-age 19 (to age 26 if full-time student): $3,750.



                           Voluntary Term Life/AD&D




                     Paid by employee on a post-tax basis.
                     Employee minimum benefit of $10,000.
                     Employee may choose increments of $10,000 up to a maximum of $500,000; not
                       to exceed 5 times annual salary, with a guaranteed issue amount of $50,000.
                     Term life benefit reduces as you age with the first reduction at age 70 to 65%, at
                       age 75 to 50% and benefits terminate at retirement.
                     Employee’s spouse may elect coverage in increments of $5,000; up to 100% of
                       employees benefit amount or $500,000 whichever is less, with a guaranteed
                       issue amount of $15,000.
                     Employee may also elect dependent child coverage in the amounts of $2,000 to a
                       maximum of $10,000.  Eligible children under 6 months of age receive $1,000.
                     Includes conversion, portability and accelerated death benefit.








                Please note, for all of Unum Voluntary Benefits, guaranteed issue is only available if you enroll when
                first eligible. If you do not enroll when first eligible you will need to complete health questions, known
                          as Evidence of Insurability (EOI), to be reviewed and approved/denied by Unum.
