Disability Benefits – Unum

Short Term Disability (STD)
In the event you are disabled due to a non-worked related accident or injury, the STD benefit amount,
if claim is approved, is equal to 60% of insured’s weekly earnings up to a maximum of $2,000 per
week.
The STD benefit is payable beginning the 1st day of a disability due to an accident and the 8th day of a
disability due to a sickness.
The maximum benefit duration is 13 weeks. Please note that there may be limited benefit durations
for certain disabilities such as maternity.
Your cost for STD coverage is based on your earnings and is reflected in the online enrollment
system, Ascentis.
Long Term Disability (LTD)
In the event that you are disabled due to a non-work related accident or injury, the LTD benefit
amount, if claim is approved, is 60% of insured’s monthly earnings to a maximum of $7,000 per
month.
If you have elected the Short Term Disability, the LTD benefit will begin (if approved for LTD claim)
once the STD maximum benefit duration has been exhausted.
The LTD benefit is offset by income from other sources such as from Social Security. The policy
includes a $100 minimum monthly benefit in the event of offsets.
The LTD Elimination period of 90 days applies (no benefits paid during this time). If you have the
STD coverage, you may receive an STD benefit (if approved) during the LTD elimination period.
The maximum benefit period is up to age 65.
The definition of disability is 24 Months based on your Own Occupation. After 24 months, you will be
subject to an Any Occupation definition of disability as per the contract.
Policy includes a 6 month look back for pre-existing conditions and disabilities that occur with the
first 12 months, due to the pre-existing conditions, are excluded (exclusion applies to new enrollees).
