Voluntary Term Life - Unum


                                                 Policy#97793



                  You have the option to elect additional term life coverage through the Voluntary Term Life
                    program.
                  For an employee, you may elect overage in increments of $10,000 up to a maximum of
                    $500,000; not to exceed 5 times your annual salary.  If enrolling as a timely entrant, a certain
                    amount of coverage is available to you without proof of good health.  This is called Guarantee
                    Issue and equals $150,000.
                  If you enroll for voluntary term life coverage, you the option to also coverage your eligible
                    spouse. Your spouse may elect coverage in increments of $5,000 up to a maximum of
                    $100,000; not to exceed 100% of employee’s voluntary life benefit amount. The Guarantee
                    Issue amount for a spouse that is enrolled timely is $30,000.
                  Covered employees may also elect dependent child coverage in the amount of $10,000 for
                    each eligible dependent children.  Eligible children under age 14 days of age receive $1,000
                    which automatically increases to $10,000 at age 15 days.
                  Includes Waiver of Premium.  If it is determined that you are totally disabled, your life
                    insurance benefit will continue without the payment of premium (subject to certain
                    conditions) up to age 65.
                  Voluntary Life policy -reduces as you age with first reduction at age 65.
                  You have options to continue coverage in the event you terminate employment - Conversion
                    privilege or Portability. If you Convert your policy when your employment ends, you may
                    apply for an individual Whole Life insurance policy during the 31 day period following the
                    date insurance terminates without having to provide evidence of insurability (proof of your
                    good health). You will be responsible for the cost of the converted policy.  If you elect to Port
                    your coverage, you policy will remain a term life policy and you will pay group rates.  For
                    either Conversion or Portability, you must make the election to continue coverage within 31
                    days of your termination.
                  Your premium is based on age and coverage elections.  Your cost is reflected in our online
                    benefits enrollment system, Ascentis.
