Voluntary Worksite Benefits - AllState
These benefits are 100% employee paid through payroll deductions. These supplemental policies
are available to help with the cost of everyday living that may accumulate in the event of a
hospitalization, a major medical diagnosis or a chronic disease or medical condition.
Accident Coverage – No one plans for an accident to happen, but they do…. on any day, at any time. The Group
Accident coverage provides a cash benefit paid directly to you and is in addition to your major medical plan
with BCBSNC. A Wellness benefit in the amount of $50 is payable once per calendar year per covered person
for receiving a wellness health screening. For a complete list of eligible screenings, please refer to the plan
booklet.
Cancer Coverage – Almost everyone knows someone that has been affected by Cancer in some way……. do
you? With the group voluntary cancer coverage, you are provided with a cash benefit for cancer and 29 other
specified diseases. You major medical coverage with BCBS does not cover the household bills that are still
piling up while you are dealing with treatments and other appointments that come along with such a
diagnosis. In most cases the wellness reimbursement benefit covers your entire annual premium cost. . A
Wellness benefit in the amount of $50 is payable once per calendar year per covered person for receiving

Critical Illness Coverage -Did you know on Average, every 40 seconds, someone in the U.S. has a
stroke? Allstate Group voluntary critical illness offers coverage that provides a lump-sum cash benefit to help
you cover the out-of-pocket expenses (this is in addition to your major medical benefits with BCBS and is a tax
free benefit when received) associated with a critical illness.
If your employment ends for any reason, you can continue this beneficial coverage. You will receive a letter in
the mail giving you instruction on how to continue paying the premium.
