Critical Illness

Full-time employees are eligible for Group Critical Illness Insurance, offered by Guardian. This
benefit pays a lump sum benefit when you are diagnosed with a critical illness. You can use the

benefits however you please, such as for medical bills, your mortgage or other bills.

You choose the level of coverage with benefit amounts of $10,000 or $20,000. If you elect spouse
coverage, your spouse is covered at 50% of your benefit amount. Children are included with
employee election. They are covered for 25% of the employee amount.

 Cancer
 Heart Attack
 Stroke
 Organ Transplant

 By-pass Surgery
 End Stage Renal Failure
 Advanced Alzheimer’s
 Advanced Parkinson’s
 Benign Brain Tumor

 Coma
 End Stage Renal Failure
 Complete blindness
 Complete deafness
 Paralysis
 Occupational HIV
Wellness Benefit: $100 is paid per year per person if a covered health screening test is

performed, including:

 Mammograms
 Colonoscopy
 Chest X-ray

 Echocardiogram
