Voluntary Life Benefits

Voluntary Life with Guardian

 Employees have the option of purchasing additional term life insurance
 Benefit amount is $50,000 increments with maximum amount of $500,000
 Guaranteed Issue amount for employee is $100,000

 Spouse benefit amount is $5,000 increments up to a maximum of $500,000,
not to exceed 100% of the employee’s amount. The rate for your Spouse’s
coverage is based on the Employee’s age.
 Guaranteed Issue amount for Spouse is $50,000.
 Child benefit is $2,500 increments for child(ren) older than 14 days with a
maximum benefit of $10,000. $1000 for child(ren) from birth to 14 days.

 Portability (if eligible) can be ported upon termination of active employment.
 Conversion –coverage may be converted to an individual policy when
coverage terminates or reduces.
