RESOLUTION BY THE ADMINISTRATION, PERSONNEL, POLICY AND LEGAL
COMMITTEE AUTHORIZING ENTERING INTO A CONTRACT WITH DAVID INSURANCE

FOR THE REMAINDER OF 2018

WHEREAS, Village Employees currently receive health insurance coverage through
Central States Health and Welfare Fund; and

WHEREAS, the Village Employees would lose health insurance coverage if the
Teamsters decertifies; and

WHEREAS, there is a need to obtain alternate insurance if the plan were to terminate;
and

WHEREAS, David Insurance has submitted a quote for health, dental, and vision
insurance that is comparable in coverage and cost to the current health insurance coverage
for village employees; and

WHEREAS, the Board of Trustees has determined that the insurance coverage
provided by David Insurance.

NOW THEREFORE, the Village Board of the Village of Sturtevant, Racine County,
Wisconsin does hereby resolve:

1. That the execution of an agreement with David Insurance for health, dental,
and vision insurance coverage for various village employees effective May 1,
2018 is authorized and approved subject to final review and approval by the
Village Administrator; and

2. That the monthly premium is set forth in Exhibit A which is attached hereto
and incorporated herein.

3. Coverage will be as provided if needed and will terminate on December 31,
2018.

4. The Village President and the Village Clerk are authorized to sign any
agreements or other documents necessary to carry out the intent of this
resolution;

Adopted by the Village Board of the Village of Sturtevant, Racine County,
Wisconsin, this 17th day of April, 2018.
