Human Resource Confidentiality Acknowledgement Form
The nature of our work often requires access to sensitive and confidential
associate information. During your assignment with Nationwide, you have a
responsibility to protect Nationwide information and must uphold the highest
standards of discretion and confidentiality and take precaution to prevent
confidentiality breaches. Nationwide expects you to adhere to the standards of
behavior described in the Nationwide Code of Conduct and Business Practices
(available for review at www.nationwide.com search “Code of Conduct”), which
is applicable to Nationwide contractors.
Examples of confidential HR information are included below. These are
examples and not an all inclusive list.
Personally Identifiable Information (PII): Social Security Number, Date of Birth, Gender,
Name, Address, Personal Contact Information (e.g., Phone, email), etc…
Job Information: Salary, Compensation, Benefits, Performance Evaluation, Succession
planning, Course results, Engagement, Job Openings/Disposition, Job Candidate
Lists/Stats, etc…
Medical Information: Medical, Absences, Disability, etc…
Diversity Information: Date of Birth/Age, Gender/Sex, Race/Ethnicity, ADA/Disability Info,
Citizenship/Visa
Associate Relations: Information obtained in connection with an associate relations
matter.
Aggregate data: Such as workforce demographics, or several data elements that when
simultaneously shared expose confidential information.
Proprietary Information: plans, programs, practices and/or policy changes that have not
yet been communicated to the majority of the Enterprise population.
Your role is to protect and safeguard confidential and proprietary information by:
Taking steps to safeguard information against unauthorized disclosure,
misuse, loss or theft.
Accessing, using, retaining and disclosing confidential information only as
needed to perform your Nationwide assignment.
Not using another contractor, consultant, agent, broker, or other third party to
do anything prohibited by Nationwide policy.
Not modifying or divulging any customer/associate information to anyone
without proper authorization and a legitimate business need to know,
Informing the authorized recipient of their role, and related business
practices, to further safeguard and protect the information.
Holding discussions of confidential nature (e.g., associate information,
associate relations issues, associate benefits, etc…) in private settings and
within the strictest confidence.
