Facilities Usage Requests
Title: Facilities Usage Request Procedures
Purpose: To have a way in which building usage can be coordinated and controlled.
Staff Assigned: Building Principals, Maintenance Director, Business Administrator, Facility
Calendar Coordinator
Procedures:
1. All facility usage requests must be on the district Facility Usage Request Form with an original
signature from applicant as per district policy.
2. Original Facility Usage Request Form will be forwarded to building Principal for approval.
3. Original Facility Request Usage Form will be forwarded to the Business Manager for approval
and charge. Business Manager will forward to Superintendent for final approval.
4. If approved form will be returned to organization/individual requesting use as permit for use.
5. Facility Calendar Coordinator will enter into calendar located on SchoolWires (district website).
6. Separate procedure for billing organizations.
7. Update facility use calendar at the end of every week.
