Requirements for Quality Control Specialist

Upgrades of Contract: In the event of a member upgrading their membership. The total
invoice that will be commissioned for personal and club goal is calculated as such: Total
remaining invoice from old contract subtracted from the total invoice of the new contract.
This total will be commissioned per your earned tier and added to club goal total for the pay
period.

Certified Personal Training Certificate: Each trainer must acquire a certified personal training
certificate from an accredited source. The trainer will have 60 days to acquire this certificate
from the signature date of the compensation plan. The certificate must be submitted to the
Human Resource department by way of scan/email. Email address is: ____________.
Failure to do so will require the employee to suspend training until the certificate is obtained.
It will not be the responsibility of Fit Physiquess to pay any employee for downtime to
achieve certifications.

Bonus pay is at the discretion of Fit Physiques. Fit Physiques reserves the right to withhold
or revoke bonus at any time.
