Special Education –
Transition Program
Our special education transition center is for young adults with special
needs to learn life and independent living skills. It is currently housed in
three mobile buildings on the Estate campus. These mobiles were last
renovated in 2006, and the program is in need of a permanent location
and upgraded facilities.
Space is available at Horizon Community Middle School but will require
adjustments and renovations to identified classrooms to accommodate
this program.
Cost: $5,000,000
The Transition Program is an optional
program for young adults with special
needs who are 18 - 21 years old, have
completed their high school core classes,
and who have been referred by a
specialized team that has determined the
need for such transition services.
Transition is the process students and
their families use to develop desired
outcomes after high school. Transition
assists with developing necessary tools
to progress from high school to adult life.
Transition may look very different
from individual to individual. Services
are provided through a “team model”
approach of educational professionals,
the student, parents, and community
resources.
Experiences and opportunities are
offered to ensure that students will be FIVE-YEAR FACILTY PLAN
able to attain individual goals and acquire
the knowledge and skills needed to
access appropriate community resources
and agencies. Transition focuses on
career and vocational development,
adult life skills, accessing community
resources/leisure activities, and
interpersonal communication.
